If you do not live within the boundary you may be eligible to be an Out of Area (OOA) patient – please see the OOA contact information sheet above. If you live within our catchment area you can register as normal.
New Patients Registration Form
You can register online using our online form or if you would prefer, you can download the forms and post your registration in.

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Postal Registrations
Please print out the GMS1 registration documents, complete in block capitals and hand in to reception.
Post these forms back to reception, but please note that if the forms are not completed properly and in block capitals or the GMS1 form is not signed this will delay your registration.
Please remember to sign the GMS1 form or you will be unable to register at the practice.
Safe Surgeries
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
