Registering with the surgery

SJWMP recognises that not all patients wish to register at a surgery close to their home address. Many patients prefer to register closer to their place of work or study. In order to address this demand, we are introducing a new type of registration called “Out of Area” (OOA) Registration. Please read our Out of Area (OOA) Contact information sheet here.

Please print out the GMS1 registration documents, complete in block capitals and hand in to reception.

Safe Surgeries

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved. Click here to view this statement in other languages.
