Registering with the surgery

SJWMP recognises that not all patients wish to register at a surgery close to their home address. Many patients prefer to register closer to their place of work or study. In order to address this demand, we are introducing a new type of registration called “Out of Area” (OOA) Registration. Please read our Out of Area (OOA) Contact information sheet here.

New Patients Registration Form

We are using an online service called Register with a GP surgery that makes it easy to register with this GP surgery. You do not need proof of address or immigration status, ID or an NHS number. The service is run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Register via NHS website

Please remember to update your NHS App when you register with us so you can continue to access your medications and medical records.

Safe Surgeries

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

NHS England has commissioned Doctors of the World UK (DOTW UK) to produce an adult and a child patient health questionnaire for refugees and people seeking sanctuary in the UK. Click here to fill in questionnaire.
